We're currently looking for an independently licensed mental health professional to join the CareNavigation team (LCSW, LMFT, LPC required) to support members seeking support withposttraumatic stress disorder (PTSD) and complex trauma.

Reporting to the Lead Specialty CareNavigator of the Trauma team, the Trauma Clinical Care Navigator will provide initial assessment,support, referral, and ongoing care coordination for members within Spring Health’s innovativeTrauma program.This is a full time position that is fully remote. The shift for this position is Monday-Friday,9-5pm pst, mt or ct.What you’ll do: Telephonic screening, brief intervention, and referral of members to a trauma specialtytreatment provider.● Apply active listening, psychoeducation, and motivational interviewing skills to understandeach member’s needs and provide support as they set goals.● Utilize Judith Herman’s phase based trauma treatment model to provide stabilizationsupport to members in their process of engaging in care.● Utilize motivational interviewing to assist members in engaging in care.● Identify an appropriate level of care, facilitate referral, and coordinate care with a SpringHealth provider.● Provide consultation regarding mental health concerns and family/relationship issues, asappropriate to member needs.● As necessary, provide in-the-moment support and safety planning for members withsafety, risks or crisis.● Utilize written communication skills to interact with members and providers through email.● Document information, maintain accurate records, perform follow-ups, and track progresswithin several systems, including an EHR within required timeframes.● Identify and problem-solve issues that serve as a barrier or disruption to care.● Interact with multi-disciplinary teams such as care support, provider operations, and

What Success Looks Like

:● Maintain high member satisfaction, as measured by survey responses● Retain members throughout their course of care by maintaining consistent outreach andcontact, as appropriate● Triage and enroll members into appropriate LOC, based on presenting problems and goals● Outreach members via phone and email to schedule triages What you’ll bring: Required mental health license: LCSW, LMFT, LPC● Minimum of 2+ years of clinical care experience working with PTSD and Complex Trauma.● Experience with motivational interviewing, diagnostic assessment, and triage.● Knowledge of phased-based trauma treatment model and EBPs for PTSD (e.g. ProlongedExposure, Cognitive Processing Therapy, EMDR, ART, etc).● Excellent listening, written and verbal communication skills.● Acknowledge and embrace diversity and inclusion.● Experience working with diverse populations.● Motivated to go above and beyond for members to provide a high-touch experience.● Tech-savvy: must be comfortable using various computer platforms and learning newsystems, and efficient in tech-related tasks. ● Able to multi-task and context switch!Bonus
